Gastric cancer peritoneal metastasis remains highly lethal, driven by complex interplay among genetic mutations, epithelial–mesenchymal transition, extracellular matrix remodeling, hypoxia–anoikis crosstalk, metabolic reprogramming, epigenetic regulation, and an immunosuppressive microenvironment. This review identifies promising biomarkers including ctDNA and exosomal non-coding RNAs, and therapeutic targets such as YAP/TEAD1, LOX, and C3-C3aR1, and advocates rational combination regimens to tackle drug resistance. Nonetheless, early detection, cellular heterogeneity, and safety concerns persist as key hurdles. This synthesis provides a practical framework for precision therapy and future investigations
